I am trying to create a firewall documentation, there are any recommendation or any template for firewall documentation.

Second, I need Ethical Hacking Report template.

There are a myriad of documentation templates for both firewall configurations and pentesting reports available on the internet, so it should be fairly simple finding examples of each.

What you're going to want to do is look through some of those examples/templates and see what makes sense to use in your environment and create your documentation with those attributes.

As for firewalls, I know a lot of people use an Excel spreadsheet or Visio to document, but I don't have a specific example of one to point you towards.
